Registering with us is quick and easy.
Check you live in the practice area for the surgery you wish to register and complete the registration forms. You can download the forms below or pop in to the Surgery and our Reception staff will provide you with the documentation you need.
Please make sure you complete the correct paperwork, every patient needs to complete a GMS1 Form, unless registering through the NHS links below; all patients also need to complete either the child’s form – to be completed for children aged birth – 5 years, or you can use the adult registration form for anyone over the age of 5 years.
If you do not want to use the GMS1 form above, or you don’t have time to pop to the surgery to collect or hand in forms; you can use the new link below – NHS – Register with a GP Surgery Service.
All patients registered with us will be allocated a ‘usual doctor’ which is then recorded on the patient record. However, patients may see any doctor within the Practice. If you wish to find out who your usual doctor is please contact Reception.
