PRIVACY AND CONFIDENTIALITY
We will respect our patient’s privacy and confidentiality.
Every person working for the NHS has a legal duty to keep information about patients confidential and not pass it on to a third party. If you want your medical records to be released to a third party you will need to complete and sign a consent form.
Staff at the Swan Surgery will not divulge patient information, including test results, dates of appointments and which doctor you have been seen by, to anyone other than the patient without the patients signed consent. This includes family members of the patient.
There are times when we have to pass on information about you to other people such as hospitals, Social Services or the Health Authority. This is always done confidentially or by removing your identifying details when they are not essential.
In order for our staff to abide by this policy you may be asked personal details to identify yourself when contacting the surgery.
We ask for information about you so that you can receive the best possible care and treatment. We keep this information, together with details of your care, to ensure that your doctor or nurse has accurate and up to date information. It also may be needed if we see you again.
