Following the link from our Home page why not try this new service. Rather than spending 5-10 mins on hold waiting for a receptionist, complete the online form which is available 24 hours a day. This questionnaire will be reviewed by one of our clinicians and you will get a response by the end of the next working day. You can also use this service for Administrative requests such as FIT notes and general health advice.
The telephone lines at both surgeries can be very busy during peak times first thing in the morning, please consider using e-Consult (see above) to save you waiting. If you are ringing to arrange a routine appointment you might find it easier to ring after 11am. If you will require a translator at your appointment, please let reception know when making the appointment so this can be arranged.

PLEASE NOTE: If you have an infectious condition, such as Chicken Pox, Shingles, Measles or you have had contact with TB please DO NOT come to the Surgery, instead make reception staff aware and you will be given a telephone appointment.
You can book a range of appointments over the internet using our online service. Click here to access this service. To register for this please speak to any receptionists at the front desk of either surgery.
When you arrive for your appointment you can either book yourself in using the touch screen or speak to a receptionist at the front desks .
All patients registered with us will be allocated a usual doctor which is recorded on their records. You can however see any of the doctors at the practice. If you wish to find out who your usual doctor is please contact reception at the surgery where you are registered
Automated Telephone Booking
This is available at Swan Surgery only and can be used to book appointments with a doctor or nurse practitioner. You are unable to book a nurse appointment through this service. You do not need to register for this and it can be accessed through selecting the relevant option from the phone menu.
If for any reason you are unable to attend your appointment, please contact the Reception Team as soon as possible so that the appointment can be made available to another patient.
